Definitely Sawako's confession to Kazehaya in Kimi Ni Todoke. It was so heartwarming and lovely. After that tiring arc of misunderstandings that was one moment that really got me. It was so cute how she kept saying
I like you and how Kazehaya pulled her inside the classroom and hugged her.
That was definitely my favourite confession scene. Ah! How I love this manga. 🙂

oh the male lead's confession in shoujo manga bitter trap (TAKAGI Shigeyoshi)
the female lead was about to jump off the window with only bed sheet covering up her body and the male lead said "I'm going to wrap you around my little finger too, until I'm satisfied so stay here. I'll make you suffer here for the rest of your life"
If we include anime, then, hands down, ef - a tale of memories. had the best scene of all times in the end:
Hirono Hiro got a call from Miyamura Miyako, who was about to leave town sad because she did not want to open up her heart to him, thinking that he liked his childhood friend, Shindou Kei, better, but was talking to him for the last time. Miyako was in a phone booth, and the card she was calling with had a time limit. This was Hiro's last chance to try and convince her to stay. As the clock was ticking, Miyako began crying:
Miyako: 'I want someone to eat my cooking while it is still warm!'
Hiro: 'I will eat anything you cook!'
Miyako: 'I want to be praised!'
Hiro: 'I will scold you when you do bad, and compliment you when you do good!'
Miyako: 'I want a person that sees only me!'
Hiro: 'I will always look at you!
I . . .!'00:00 Beep
The phone hangs up, and Miyako falls to her knees crying.
When all hope seems lost, a voice is head:
'I love you!'
Hiro is panting on a bicycle:
'So you were here . . . Hah . . . I have looked in every phone booth in town to find you.'
Then they kiss and live happily ever after; it was such a great series.

I don't know if this counted as a confession but chapter 13 of Dominanodo! certainly was unforgettable in a the same way the confession in "Misery" was. 🤢

Yeah this one was pretty good lol...
Unfortunately the anime adaptation failed compared to the manga.
And I 5th or 6th (or w/e number it's on) the FMA one.
The confession I hate most though is shoujo's "I will make you fall in love with me so just wait" confession.
